Inspector Notes
An unannounced monitoring inspection was initiated on 06/03/2025 and completed on 06/04/2025, in response to a self-report received by the licensing office on 05/28/2025 relating to programs. The inspector reviewed 1 child?s record and 2 staff records on-site.
This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/11/2025. A POC submitted after this date will not appear on the public website.
Violations
2The center shall be in compliance with the center?s own policies and procedures that are required by the standards.
The center?s required discipline policy states that striking a child and roughly handling a child is not permitted. On 05/28/2025, Staff 1 roughly handled Child 1 (age 4 years) and struck Child 1 five times with a pencil.
The center was previously cited on 04/09/2025 for not following the center?s own discipline policy.
The following actions or threats thereof are forbidden: striking a child or roughly handling a child.
On 05/28/2025, the following forbidden actions occurred during a period of approximately 5-7 minutes: Staff 1 grabbed Child 1 (age 4 years) around the waist and pulled the child down onto their cot. Staff 1 grabbed Child 1 by the left arm and jerked the child down onto their cot. As Child 1 was standing by their cot, Staff 1 grabbed Child 1 by their left arm and pulled the child across the classroom.
After returning to their cot, Child 1 stood on the cot with their hands on the half wall of the classroom. Staff 1 struck the child on their upper right arm, which was covered by a shirt sleeve, with a pencil five times. Following the final strike of the pencil, Child 1 immediately took their hands off the wall and walked away rubbing their arm.
The center was previously cited on 04/09/2025 for using a forbidden action.
