Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/26/2025. The on-site inspection began at 11:00 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

The TB screening for Staff 2 was completed more than 2 months prior to employment.
Plan of Correction: Will no longer accept TB screenings that have not been obtained within 30 days.
Standard 8VAC20-780-500-A
Staff shall wash their hands with soap and running water before preparing or serving food or beverages.

Staff 2 did not wash their hands before preparing a bottle.
Plan of Correction: Will wash hands before preparing or serving foods and beverages.

At least one staff member in each classroom or area where children are present shall have a current CPR and first aid certification from an organization that includes an in-person competency demonstration.
