Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A subsidy health and safety (SHSI) inspection was initiated on 8/18/2021 and concluded on 8/19/2021. There were 28 children present with 9 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain documentation completed within the 30 days prior to employment for each staff stating that they are free of communicable tuberculosis (TB).

Evidence: Staff #1 (DOH: 7/19/21) had a TB test on file dated 5/7/21. Staff #5 (DOH: 5/4/21) had a TB test on file dated 4/2/21.
Plan of Correction: Staff #1 (hired 7/19/21) received a second negative TB result on 8/19/21. Staff #5 (hired 5/4/21) received a second negative TB result on 8/14/21. Moving forward, all staff will obtain negative TB/PPD results within the required 30 days prior to start date.
