Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was conducted on 3/3/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 42 children present with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on medication review, the center did not obtain parental authorizations for all medications on site.

Evidence: Child #6 had two emergency medications and Child #7 had one emergency medication on site. Parental authorizations granting the center permission to administer the emergency medications were not on file for Child #6 and Child #7.
Plan of Correction: Parents of Child #6 and Child #7 corrected the school name on the medication forms on site. Moving forward, when children transfer from sister locations, new medication forms will be completed for the current school.

Based on record review, the center did not ensure that two or more references as to character and reputation as well as competency were checked before employment.

Evidence: Staff #2 (Date of Hire: 11/29/2021) had reference checks on file that were completed 11/30/2021.
Plan of Correction: References for potential employees will be completed prior to start date.
