An unannounced monitoring inspection was initiated on 07/01/24 and completed on 07/10/24 in response to a self-report received by the licensing office on 06/28/24 relating to supervision. The inspector reviewed one child?s records and one staff records on-site on 07/10/24. This inspection included document review, tour of the facility, interviews, and observations.
