When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 11/03/25, Child #1 (2-years-old) was left behind in the classroom when the rest of the class went to the playground.
Staff #3 was in the Toddler Explorers classroom when she heard muffled crying coming from the room next door. Staff #3 looked in the room and observed Child #1 was alone, on the ground, crying, and alerted a member of management. It is estimated Child #1 was left behind for approximately 8 to 10 minutes.

Staff #3 was in the Toddler Explorers classroom when she heard muffled crying coming from the room next door. Staff #3 looked in the room and observed Child #1 was alone, on the ground, crying, and alerted a member of management. It is estimated Child #1 was left behind for approximately 8 to 10 minutes.
Plan of Correction: Per the Center: "Upon being alerted, management responded immediately and ensured the child was reunited with the group and assessed for well-being. The parents were called and notified of the incident. The incident was discussed in more detail with the parent that afternoon at pickup and again the next morning when the child arrived at the center. The involved staff were addressed regarding their failure to maintain proper supervision and to follow required procedures during transitions. Appropriate corrective action was taken in accordance with center policy. Following the incident, the center conducted a mandatory Name-to-Face accountability training for all staff. Staff were required to explain and role-play the procedure for their classroom, with emphasis on:
Maintaining supervision of all children at all times, completing Name-to-Face checks prior to leaving any area, performing a full room sweep before exiting, repeating Name-to-Face checks after each transition. Staff were reminded that children must never be left unattended, and that supervision is required at all times, including during transitions and line-up procedures. Administrative staff will continue to monitor classroom transitions and supervision practices to ensure compliance. Any future failure to maintain supervision will result in additional corrective action."

A mandatory Name-to-Face procedure for their classroom, demonstrate how sight and sound supervision is maintained during transitions, reinforce that counting children does not replace Name-to-Face verification, identify required checks before leaving any space and upon arrival at the destination.
Staff were reminded that failure to maintain sight and sound supervision at any time is unacceptable and will result in corrective action, up to and including termination.
Administrative staff will continue routine monitoring of transitions and supervision practices to ensure children remain within sight and sound supervision at all times."

The center is required to ensure the center's own policies and procedures that are required by these standards are in compliance.
The center's procedures for identifying where children are at all times states: "Name to face accountability is when you look at your rollcall sheet and confirm by actually putting your eyes on each individual child marked present on the rollcall sheet. This procedure does not take the place of counting the number of children as you cross each threshold on a regular basis. However, children are busy and constantly moving around which makes it easy to count the same child twice. All areas should be checked prior to transitioning."
On 11/03/25, Staff #1 and Staff #2 did not follow the center's Name-to-Face procedure when transitioning children outdoors and did not check all areas of the classroom before transitioning. The staff acknowledged counting the children when they lined up to go outside, but a count and the name-to-face check was not done again when the children were out on the playground. Staff reported a final check of the room was not completed when the group exited the room and Child #1 (2-years-old) was left behind for approximately 8 to 10 minutes.
