Inspector Notes

A monitoring inspection was initiated on June 22, 2022 and concluded on June 22, 2022. There were seven children present, ranging in ages from Infant to Three years of age, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
The record for child #1 (start date: 8/18/21) did not have documentation of a physical examination on file.

Standard 8VAC20-800-540-A
Based on observation and interview, the provider did not ensure that when a child is placed in a feeding or activity table, the protective belt shall be fastened securely.

Evidence:
1. Six children were seated at the feeding or activity table and six children did not have a protective belt securely fastened.
3. The children in care were infants, toddlers and preschoolers.

Based on record review and interview, the provider failed to obtain verification of identity within seven business days of a child's first day of attendance for one out of three children.

Evidence:

2. The provider stated that the proof of birth had not been reviewed for child #2.
