During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated on 8/21/2025 and completed on 8/21/2025. The on-site inspection began at 12;40pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was thirty. The inspector reviewed five children?s records and two caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #4, who had been in care for approximately six months.

Standard 8VAC20-800-140-B-3
At least one caregiver in the home where children are present shall have current certification in both CPR and First Aid.

Caregiver #1 and Caregiver #2 did not have up to date certifications in CPR. Staff #1 CPR certification expired 03/20/2025.

Within seven business days of the child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child and name and location of previous day care programs.
