An unannounced inspection was conducted on November 8, 2023 from 11:30 a.m. through 12:00 p.m. This inspection was initiated from a complaint that was received by the Valley licensing office.
Documentation was reviewed and interviews conducted. The information gathered during the investigation supports the allegations, therefore, the complaint is determined to be valid.

Based on record review and interview, the facility failed to follow their dismissal plan to ensure safety of children.
