Inspector Notes

The licensing inspector initiated an unannounced remote inspection on 11/15/2022 and concluded on 11/15/2022 in response to a complaint received by the department on 11/10/2022 for allegations regarding programs. Interviews were conducted with the staff relating to the allegations. A total of 3 staff records were reviewed/updated. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid". There was one violation cited not related to the allegation. The exemptions to compliance are noted on the violation notice.

Based on record review, the provider did not obtain a criminal history record check prior to the 1st day of employment.

Evidence:
1. Staff #3's file contained a fingerprint background check that was over ninety days old from date of rehire. The date of rehire is 09/22/2021. The date of the fingerprint criminal history record check in the file for Staff #3 is 08/28/2020.
2. Administration confirmed the date of rehire and the date of the fingerprint criminal history record check that was in the file.
Plan of Correction: When rehiring staff, director will be sure to get fingerprint background checks completed prior to rehire.
