Inspector Notes

A monitoring inspection was initiated on 06/27/2023 and concluded on 06/27/2023. There were 69 children present, ranging in ages from 6 weeks to 11 years, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the center failed to ensure that a negative tuberculosis test/screening was submitted at the time of employment and prior to coming into contact with children.

Evidence:
1. The record of Staff 1 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 1 was working in the classroom when the Licensing Inspector was completing observations.
2. The record of Staff 2 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 2 was working in the classroom when the Licensing Inspector was completing observations.
3. The record of Staff 3 (DOH 06/15/2023) did not have documentation of a negative tuberculosis test/screening result. Staff 3 was working in the classroom when the Licensing Inspector was completing observations.
Plan of Correction: I thought we had time to get it after they were hired. I will get them to get the TB screenings this week.
