During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 3/20/2025. The on-site inspection began at 9:50 am and ended at 12:36 pm. The inspector reviewed compliance in the areas listed above. There were 40 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/27/2025.
A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff #2, who is currently working, was dated 5 days after their first day of employment. Staff #2 completed orientation and training but did not work with children prior to the completion of the sworn statement.
Plan of Correction: As noted, the sworn statement for staff #2 was completed prior to her beginning classroom duties.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

The fingerprint background check for Staff #2, who is currently working, was dated one month after their first day of employment. Staff #2 completed orientation and training but did not work with children prior to the completion of the background check.
Plan of Correction: As noted, the criminal background check was completed prior to staff #2 beginning classroom duties.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry, a criminal history name check and a sex offender check from one state for Staff #1, who has been employed for one week, and was working with children at the time of the inspection.
Plan of Correction: The sex offender check and criminal history name check have been requested. Staff #1 has been asked to submit a search of the child abuse and neglect registry.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

The screening for staff #2 was completed 5 days after the start of employment. Staff #2 did not work with children prior to the submission of the negative TB screening.
Plan of Correction: The TB screening was completed prior to staff #2 beginning classroom duties.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Staff #1, who has been employed for 1 week, did not have any completed reference verifications on file.
Plan of Correction: References will be contacted and the information will be documented.
