Inspector Notes
An unannounced inspection was initiated on 2/28/2024 in response to a self report that was received by the licensing department on 2/9/2024 regarding a serious injury that occurred at the center. The inspection was initiated on 2/28/2024 and completed on 3/1/2024. On 2/28/2024, the inspector was on site from approximately 9:08-9:55 am. An interview with staff was completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted remotely.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
