During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 10/08/2025. The on-site inspection began at 9:30 am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 45 children present and 8 staff. The inspector reviewed 5 children?s records, 5 staff records, and 2 officer records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/15/2025. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry from one state for Staff #4, who has been employed for more than a month and was working alone with children at the time of the inspection.

checks were originally submitted
and returned as ?eligible for
employment?, and the Florida
Background Investigation office
informed us we completed
everything needed for
background checks, the Child
Abuse and Neglect Registry is a
separate and 3rd background
check needed, which has now
been submitted.
Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2, who has been in attendance for one month, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: A completed and signed MAT
form was on hand and a new
Care Plan form has been given
to the parent for completion.
