Inspector Notes

A renewal inspection was initiated on-site on December 14, 2021 and concluded on December 15, 2021. There were 18 children present, ranging in ages from 16 months to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-270-A
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: 1. Each classroom in the center had several areas of chipped and peeling paint. The areas were accessible to the children. 2. The 2yr old classroom had approximately 5 screw holes in the walls. 3. The boy's bathroom had large areas of the stall walls covered with rust. The rust was rough to the touch and could cut children's clothing and/or skin.
Plan of Correction: Bathroom stall will be sanded and repainted this weekend. Chip paint in classroom will be spackled. Entire building will be repainted next month. Moving forward the Director will make sure that the building will have an annual paint refresh, or as needed throughout the year.

Based on a review of records, the center did not ensure that each child's record contained the required information.
Evidence: The record of child #1 did not contain documentation of a second emergency contact person to contact if a parent cannot be reached.
Plan of Correction: Advised assistant on yesterday that parent can not be emergency contact. Mother has already provided another emergency. Director will review all folders of new enrollees.
