Inspector Notes
A complaint investigation was initiated on 11/05/21 and concluded on 02/08/22 in response to a complaint that was received by the department on 10/26/21 in the areas of toileting procedures and preventing the spread of disease. The inspector conducted an unannounced inspection on-site on 11/23/21 from approximately 11:37am to 3:20pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There are no violations resulting from this inspection.
