Inspector Notes

The licensing inspector conducted an unannounced monitoring inspection on Wednesday, May 4th, 2022 from 10:45am to approximately 3:15pm. There were a total of 109 children in care in the direct care of 17 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility including classrooms, kitchen, hallways, playgrounds, and bathrooms were inspected. The center is equipped with toys and supplies and items were available to the children. The center provides transportation and buses were inspected for compliance. The required postings were reviewed and found to be in compliance. Medication is administered when required and medication and medication authorizations were reviewed. During the inspection, 10 children?s records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interviews, the center that was built before 2015 and serves preschool-age children is not equipped with at least one carbon monoxide detector.

Evidence: 1) The center was not equipped with at least one carbon monoxide detector. 2) During interviews, a member of management confirmed the center did not have a carbon monoxide detector on site at the time of the inspection.
Plan of Correction: A carbon monoxide detector will be installed at the center.
Standard 8VAC20-780-270-A
Based on observation on 05/04/22, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: 1) During the inspection, the inspector observed a portion of the carpet wall in the School Age gym that was ripped. 2) In the Private-K classroom, the inspector observed a food stain on a cabinet door under the sink. The stain had been previously observed during a previous inspection. A member of management and staff were observed cleaning the food stain off of the cabinet during the inspection.
Plan of Correction: A request will be made to fix the ripped areas of the carpet wall.
Standard 8VAC20-780-280-B
Based on observations at the center on 05/04/22, the center did not ensure to keep all hazardous substances such as cleaning materials in a locked place using a safe locking method that prevents access by children.

Evidence: 1) The licensing inspector observed bottles of bleach/water solution on a metal shelf in the Green birds 2?s bathroom. 2) The licensing inspector observed an unlocked cabinet with cleaning supplies in the School Age gym.
Plan of Correction: Corrected on site.
Standard 8VAC20-780-560-M
Based on observations, the center did not ensure children were not allowed to drink or eat while walking around.

Evidence: 1) During the inspection, the inspector observed a staff member offer the children in the Orange Birds 1 classroom their water bottles after they arrived inside from the playground. The children were observed drinking water and walking around in the classroom.
Plan of Correction: Corrected on site. Staff were reminded to ensure children are not allowed to eat and drink while walking around.
