Inspector Notes
A complaint investigation was initiated on 03/02/2022 and concluded on 05/18/2022 in response to a complaint that was received by the department on 03/02/2022 regarding allegations in the areas of care and protection; and forbidden action by staff. Two inspectors conducted an unannounced inspection on-site on 03/31/2022 from 10:15am to approximately 11:45am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. A follow-up unannounced inspection was conducted at the facility on 05/04/2022 from 10:45am to approximately 3:15pm where additional interviews and documentation was reviewed. On 05/18/2022, two inspectors conducted an unannounced inspection on-site to conduct additional interviews from 11:00am to approximately 12:48pm. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There are no violations resulting from this inspection.
