Inspector Notes

A complaint investigation was initiated on 02/16/2022 and concluded on 03/31/2022 in response to a complaint that was received by the department on 02/16/2022 regarding allegations in the areas of ratios, supervision, sanitation, food services, and the physical plant. The inspector conducted an unannounced inspection on-site on 03/15/2022 from 7:45am to approximately 8:42am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. A follow-up unannounced inspection was conducted at the facility on 03/31/2022 from 10:15am to approximately 11:45am where additional interviews and observations were made at the center. The preponderance of the evidence gathered during the investigation supports part of the allegations, therefore the complaint is determined to be valid. Based on observations and inspection of the facility, the center did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: During the inspection conducted on 03/15/2022, the licensing inspector observed a bottle of cleaning solution and shaving cream on a shelf in the Greenbirds bathroom.

Based on observation and interview, the center did not ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents.

Evidence: 1) During the inspection conducted on 03/15/2022, the licensing inspector observed a bottle with a clear substance that was not labeled with its contents in the Greenbirds classroom. 2) During interview, a staff member verified that it was a bleach-water solution.
