Inspector Notes
An unannounced monitoring inspection was conducted on March 1, 2023 with center director. The inspection began at approximately 9:30am and concluded at 1:30pm. There were 70 children present, ranging in ages from 0 to 5 years, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction, and date to be corrected for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Violations
5Evidence:
1. Staff #5?s record did not contain documentation of a completed orientation.
2. The director verified that staff #5 completed orientation but was unable to provide documentation.
Evidence:
1. 1 of 2 sinks are operable as the water faucet is broken in the Green Bird?s 1 classroom.
2. In the private kindergarten room, there is a red chair and the vinyl is torn (approximately 3 1/2 inches long) and the inside foam is exposed.
4. In the Green Birds 2 classroom, a red chair is torn (approximately 1-2 inches long) exposing the foam.
5. A playhouse that was located between the preschool and school age playground, had a broken handle that exposed a sharp edge that was approximately 1- 11/2 inches long.
Evidence:
1. In the Younger Four?s Classroom there was a container of Clorox Disinfectant Wipes sitting on a shelf that was accessible to children.
2. The warning label stated hazardous to humans and animals and keep out of reach of children.
Evidence:
1. The licensing inspector observed staff in the Baby Birds II classroom, changed the diaper of one child. After the child was changed, the staff sat the child down on the carpet. The child?s hands were not washed after diapering.
2. The licensing inspector observed that during diaper changes that staff did not clean the diaper changing table with soap and water, or follow-up with sanitizing the surface between diaper changes.
Evidence:
1. Child # 2?s record (start date 9/9/2022) was missing the physicians name and phone number.
2. Child #4?s record (start date 9/27/2022) was missing the physician?s name and phone number.
