8VAC20-780 Physical Plant.

During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 8/13/2025 in response to a complaint received by the licensing office on 7/22/2025 relating to allegations of inadequate building maintenance. This inspection included a tour of the school age rooms, observations and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/26/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. On 8/13/2025, the floors and carpets in the gym area and the two school age classrooms were observed to be dirty and did not appear as if they had been swept, vacuumed, or mopped recently. There was dirt, dust, hair, food, bits of trash, and debris on the floors and carpets where children were playing. The tables and desks in the two school age classrooms were smeared with a buildup of fingerprints, smudgy handprints, and bits of food and trash. The trashcan in one of the school age rooms was overflowing with garbage.
Plan of Correction: Reviewed with staff when and how to clean and sanitize, policies and procedures, before and after each meal, as needed; when to take out trash and sweep, mop, vacuum; cleaning crew came and will wax floors in September. Floors will be stripped and waxed in September (first available); continue cleaning practices as described and as needed.
