Inspector Notes
An unannounced renewal inspection was conducted today between 12:25pm and 1:45pm. There were 8 children present (22 points) with the Provider and 2 assistants within the supervision guidelines. The children were observed taking a nap and being held. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

51. Caregiver #1 only had 2 of the 16 required annual training hours documented.
2. Cargiver #2 had 0 of the 16 required annual training hours documented.
3. Caregiver #3 had 0 of the 16 required annual training hours documented.
Evidence:
1. Upon arrival to the family day home, Child #1 was observed sleeping on the floor with no cot or rest mat.
Evidence:
1. Child #1 was missing documentation of the Emergency Medical authorization and the Emergency Preparedness Plan during the time of the inspection.
2. Child #2 was missing documentation of the Emergency Medical authorization, physical, and immunizations during the time of the inspection.

1. The battery-operated weather band radio was not in working order during the time of the inspection.

1. The emergency preparedness plan was last reviewed on 1/27/2019, according to documentation reviewed during the time of the inspection.
