Inspector Notes
An unannounced monitoring inspection was conducted 3/7/2024 from approximately 9:35am-10:30am. There were 7 children (15 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed sitting at a table coloring, practicing letter recognition, singing and handwashing was also observed. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

Evidence:
Child #1 and Child #4 are missing signed written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
Evidence:
Child #4 missing written parent authorization for diaper ointment that caregiver stated, she has used on the child.
for a child who is not exempt from the immunization requirements, once every six months for children under the age of two years.

Child #3s (22 months old) record last immunizations on file 8/23/2023.
