Inspector Notes

An unannounced, on-site renewal inspection was conducted on 9/12/2024, as a part of the licensure period. The on-site inspection began at 12:15pm and ended at 1:17pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 7. The inspector reviewed 3 children?s records and 3 caregiver records on-site. This inspection included document review of policies and procedures, emergency drill logs, authorization forms, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Based on record review, the provider did not have all required information in child's record.
Evidence: Child #3's record di not have written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately or
written objection to provision of medical treatment on religious or other grounds.
Plan of Correction: Provider will request parent's signature on authorization at pick up 9/12/24.
Standard 8VAC20-800-90-C
Based on record review and interview, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization; once every six months for children under the age of two years.
Evidence: Child #2 DOB 4/2022, date of last immunization 1/2024.
Plan of Correction: Provider to request updated immunization record from parent 9/12/24 at pick up.
