Inspector Notes

Thank you for your time and assistance with the unannounced monitoring inspection conducted on June 16, 2022. The on-site inspection was held from 12:05 p.m. to 2:00p.m. On this date, there were 21 children present in four classrooms and in the care of five staff members. All four classrooms were toured and observations were made in one of those rooms. Four staff members' records and three children's records were reviewed/updated. The lunch served met the USDA guidelines and consisted of hamburger, peas, mandarin oranges, and milk. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Some items reviewed/observed: required posted information, documentation of evacuation and shelter-in-place drills, first aid kit and emergency supplies, infant care, diaper changing procedures, hand washing, lunch, and nap time.

Based on review of staff records, the center failed to obtain an updated criminal history record check prior to employment.

Evidence:
1. The record for Staff A contained a criminal history record dated 03/09/2018. Staff A's date of rehire was 01/07/2021.
2. Administration verified that FP background check was not updated when Staff A was rehired.
Plan of Correction: I will be sure to make staff get new fingerprint background checks if I rehire them. I will get her to get her fingerprinting completed as soon as possible.
