Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on December 14, 2022 from 1:30 p.m. through 3:00 p.m. Upon arrival, there were 19 children, ages 4 months through 5, in the care of 5 staff. Three children and six staff records were reviewed and updated. No medication is being administered at this time.

Areas observed were: hand washing/bathroom procedures, nap and snack. Interactions between the staff and children were observed as positive. The center is well equipped with a variety of age/stage materials for the children in care.

Areas of non-compliance may be found on the violation notice. Please complete your plan of correction and return to the licensing inspector within 5 business days.

Based on observation the center failed to have the required amount of resilient surfacing around the fall zones of the outside slides.

Evidence:

1. The outside playground was toured.
2. Less than 1 inch of resilient surfacing was observed at the base of the blue and green slide.
3. The director confirmed there was less than 1 inch of resilient surfacing at the base of the slides.

Based on observation the center failed to have a nonabsorbent surface for diapering in one classroom.

1. The diapering changing pad was observed in room #4.
2. The changing pad was observed to have a small tear making it nonabsorbent.
3. The director confirmed the changing pad had a small tear.
