Inspector Notes

An unannounced monitoring inspection was conducted on June 11, 2024 from 11:00 a.m. through 12:00 p.m. Upon arrival, there were 20 children, ages 10 months through 5, in the care of 5 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 children and 1 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on observation the center the center failed to maintain the wood frame of the outside doorframe in a safe condition.

Evidence:

1. The wooden doorframe on the outside playground was observed to be chipped and visible peeling paint .
2. Administration confirmed the doorframe was peeling paint.

Based on observation the center failed to maintain resilient surfacing under the bottom of the slide on the outside playground.

Evidence:

1. Mulch was measured to be less than 1 inch at the bottom of the outdoor slide.
2. Administration confirmed mulch was not measuring 6 inches or greater.
