Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on August 25, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were nine children(23pts) present, ranging in ages from 5 months old to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-180-A
Based on record review and interview, the provider did not obtain a subsequent Tuberculosis Screening for each caregiver.
Evidence:
1. Staff 1 and Staff #2 records contained expired tuberculosis screenings. Staff #1's last tuberculosis screening was dated 05/1/2019. Staff #2 last tuberculosis screenings was dated 05/01/2019.
Plan of Correction: The provider has made appointments for tuberculosis screenings and will put the results in each record.
