Inspector Notes

An unannounced renewal inspection was conducted today between the hours of 11:30 am through 1:00 pm. There were a total of 11 children (23 points) observed with two caregivers. A total of five children's files and four staff/household member's files were reviewed. The children were engaged in the following: toy play, diapering, bathroom breaks, handwashing, lunch time, and nap time. Areas of non-compliance are identified in the violation notice. Based on record review and interview, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child # 4's record (Start date-03/21/2022) did not contain documentation of a physical examination.
2. Caregiver #1 confirmed that Child #4's physical examination was not obtained from the parent.
Plan of Correction: The provider will get a copy of the physical examination from the parent once the child goes to the physician.
