Inspector Notes

An unannounced monitoring inspection was conducted today from 9:00am-10:00am. There were 4 children directly supervised by the provider. The home, 2 caregiver records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed having breakfast and playing freely. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Repeat violation

Based on record review and provider interview, the provider did not obtain a search of the central registry within 30 days of a person 18 years or older beginning to reside in the home for 2 of 2 household members.

Evidence:
1. The provider stated Household Member (HHM) #1 began residing in the home in November 2023. The record for HHM #1 did not contain documentation of the results of a search of the central registry.
2. The provider stated HHM #2 began residing in the home in March 2024. The record for HHM#2 did not contain documentation of the results of a search of the central registry.
Plan of Correction: Requests were submitted last week.
Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain a tuberculosis screening every two years for 1 caregiver.

Evidence:
1. The most recent documentation of a tuberculosis screening in the record for Caregiver #2 was dated 09/21/2021.
Plan of Correction: The caregiver will schedule an appointment.
