During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on October 17, 2025, as a part of the licensure period. The on-site inspection began at 9:45 a.m. and ended at 10:45 a.m. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 3 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the home, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Household Member #2 was not completed and was overdue by over one month.
Plan of Correction: The household member will complete the TB test this weekend.
Standard 8VAC20-800-60-B
The provider shall ensure an up-to-date record is maintained for each enrolled child with the required information.

The record for Child #3, enrolled for over 4 months, was missing the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached, and the first date of attendance.
Plan of Correction: The required information will be obtained from the parents.
Standard 8VAC20-800-70-A
The provider shall ensure parents are provided in writing the required information prior to a child's first day of attendance.

The record for Child #3, enrolled over 4 months was missing the requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up, and the requirement for the parent to inform the family day home within 24
hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease.
Plan of Correction: The required information will be obtained from the parents.
Standard 8VAC20-800-830-A
The provider shall ensure emergency evacuation procedures shall be practiced monthly.

The last documented emergency evacuation drill was over 3 months ago and the provider confirmed one was not completed since.
Plan of Correction: Emergency drills will be practiced monthly moving forward.
