Inspector Notes

A virtual monitoring inspection was initiated on June 10th and concluded on June 24, 2022. The site director was contacted by telephone to initiate the inspection. There were children present ages infant through four years old with 13 staff. The inspector emailed the site director a list of items required to complete the inspection. The Inspector reviewed three children?s records and three staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Based on observation the center failed to ensure that when an infant is able to easily turn over from the back (supine) to the belly (prone) position and he is placed in his crib, he shall still be put on his back (supine) but allowed to adopt whatever position he prefers.
Evidence:
Staff 1 stated that she placed child 1 in his crib on his stomach.
Plan of Correction: This violation was corrected immediately as staff were reminded and retrained on the requirements for all infants to be placed in cribs on their back.
