The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #4, under two years old has been in care for 7 months did not have additional immunizations in their record.
Plan of Correction: Response from the director:
Management will ensure a new shot record is obtained every six months for all children under 2 years of age even when no additional shots are required for a child during that time period.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.
