Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on March 28, 2022 and concluded on March 31, 2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 60 children present, ranging in ages from 18 months to age 5, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on review of 4 staff records and staff interviews, the licensee did not ensure any employee has obtained a central registry (CPS) finding within 30 days of employment.

Evidence: The record for staff #1 date of hire 2-15-2022 did not include the results of a CPS finding.
Plan of Correction: Staff #1 CPS findings is complete. CPS findings will be returned prior to working.
Standard 8VAC20-780-160-A-1
Based on review of 4 staff records and staff interviews, the licensee did not ensure employees have obtained documentation of the TB screening and it shall be submitted at the time of employment and prior to coming into contact with children.

Evidence: The record for staff #2 includes as TB test / screening dated 11-25-2021 and documented first date for working with children is 11-22-2021.
Plan of Correction: Staff #2 has TB screening. Screenings will be completed prior to beginning work.

Based on review of 4 staff records and staff interviews, the licensee did not ensure employee TB screening / documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: The record for staff #4 includes a TB test / screening dated 10-26-2020 and documented first date for working with children is 08-30-2021.
Plan of Correction: Staff #4 has TB screening. Screenings will be completed prior to beginning work.
