Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, document review, and an interview with staff.

A monitoring inspection conducted on 8/27/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 57 children present, with eight staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programming, special care provisions and emergencies and special services. A total of four child records and four staff records were reviewed.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22VAC40-185-160-C
Based on review of four staff records, and an interview, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the initial screening or testing. Evidence: the TB test/screening for staff #1 is dated: 8/6/2019 and for staff #2 is dated: 6/21/2019.
Plan of Correction: Staff stated that staff #1 and staff #2 will be directed to get an updated TB test/screening.
Standard 22VAC40-185-350-E-2
Based on review of documentation, and an interview with staff, the facility failed to maintain the ratio of one staff for every 5 children, ages 16 months through two years. Evidence: On 8/17/2021 there were eight children present in the prek-2 classroom with one staff supervising. The youngest child in the room, with a date of birth: 10/14/2019, was 22 months old.
Plan of Correction: Staff stated that they had verbal permission from the family to maintain the higher ratio, and will obtain written documentation for the child's record indicating this.
