Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Staff A Tb test was due to be repeated. TB has expired for approximately four months.
Staff B Tb test was due to be repeated. TB has expired for approximately four months.

Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Staff A is missing 11.5 of 16 required hours of training for 2025.
Staff B is missing 11.5 of 16 required hours of training for 2025.

Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

There were 2 outlets without covers behind the cribs.
Plan of Correction: There is paint in the outlets. You cannot get covers in.
