Inspector Notes

A monitoring inspection was initiated on January 18, 2024 , at approximately 10:25 AM to 11:30 AM. There were six preschool children present with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. A total of six child records, two household member records, and two provider records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Based on observation and interview, the provider failed to ensure that trampolines shall not be used during the hours children are in care.

Evidence:
The provider stated that the children use the small trampoline that is located in the back yard.
Plan of Correction: The small trampoline was removed from the backyard during the inspection.
Children not outside at the time, staff has all been notified not to leave out for personal use.
