Inspector Notes
An unannounced monitoring inspection was conducted on 6/5/2024. The inspector was on site from approximately 9:55 am-11:30 am. There were 6 children in care, ranging in age from 3 to 4 years, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, care of children, emergencies, caregiver training, preventing the spread of disease and nutrition. A total of 5 child records, 2 caregiver records, and 3 household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations

1. The record of household member #3 (DOB:4/10/2010) did not contain a central registry finding.
2. The provider acknowledged that a finding had not been obtained within the required time period.
Evidence:
1. The water tap at the children's handwashing sink was measured twice and both readings registered at 124?F
2. The provider acknowledged that the water temperature was above 120?F.
Evidence:
1. The refrigerator contained a thermometer that read 50?F.
2. The provider acknowledged that the temperature was not at or below 40?F.
