Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated and concluded on 12/06/2021. The supervisor was contacted by telephone and a virtual inspection was conducted. There were 59 children present, ranging in ages from 4 months old to 5 years old, with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.

Based on review, the center did not have docu- mentation of the fingerprint criminal history record check results for one staff member.
Evidence:
1.Staff member #5 does not have fingerprint criminal history record check results for this center.
Plan of Correction: Fingerprint criminal history check specific for Sparkles! at Crystal City will be requested immediately and submitted as soon as the documentation is received.

Based on review of four staff records, the center did not obtain a central registry report within 30 days of employment.
Evidence:
1. Staff #1 (Date of hire 10/04/2021) and Staff #2 (Date of hire 10/18/2021) did not have the central registry results within 30 days of employment.
Plan of Correction: I will follow up with them today.
