Inspector Notes
An unannounced monitoring inspection was conducted on 1/24/2023 from 10:07am to 12:10pm. At the time of entrance, 62 children were in care with 18 staff members present. Children were observed singing B-I-N-G-O, eating snack, washing hands, playing at the sand table and playing with containers, playing with table toys, mixing paint colors, watching the teacher perform a finger puppet show, listening to music and dancing. The infants were playing with toys on the carpet with their teachers, being held and drinking bottles, diaper changes, hand washing and sleeping. Interactions between the children and staff were positive. The site was organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, the physical space, evacuation drills, emergency supplies and attendance records were reviewed. 1. Staff #1 (Start date 12/27/2022) lived in DC in the last 5 years and the center did not obtain the criminal history record for DC until 1/04/2023.

1. Child #1 (start date 11/02/2020) had an immunization record, but it was not signed by a physician, his designee, or an official of a local health department and it was not on the current form required by the Virginia Department of Health or a physician's form.

1. Staff #3 (start date 10/10/2022) and Staff #4 (start date 10/18/2022) had not completed the Virginia Department of Education-sponsored orientation course.

1. In the Preschool 1 classroom, a spray bottle of Lysol and Clorox wipes were in a lower cabinet and the cabinet was not locked.
