Inspector Notes
An unannounced monitoring inspection was conducted on 10/16/2023 from 10:15am to 12:25pm. At the time of entrance, 44 children were in care with 19 staff members present. Children were observed playing with toys, leaving to go on a walk to find leaves, listening to a book recording, singing and dancing. Infants were returning from a buggy ride, being held and fed a bottle, sitting in bouncy seats and playing on the floor with their teachers. A selection of (4) staff and (3) children records, medications, the physical space, evacuation drills, allergy action plans and attendance records were reviewed. The site was clean and organized and contained an abundant supply of developmentally appropriate materials. Bulletin boards and several other areas of the programming space was decorated with artwork and projects completed by the children. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations
4Based on review of staff records, the center did not obtain the results of the criminal history
record information check or the Sex Offender registry check from any state in which the individual had resided in the preceding five years prior to the first day of employment.
Evidence:
1. Staff #2 (Hire date 2/13/2023) lived in DC in the last 5 years and the center did not obtain
the criminal history record for DC or the Sex Offender Registry Check for DC prior to hire. The center received the Criminal History Check on 2/22/2023 and the Sex offender registry check on 3/13/2023.
2. Staff #3 (Hire date 1/04/2023) lived in DC in the last 5 years and the center did not obtain
the criminal history record for DC prior to hire. The center received the criminal history record on 3/06/2023.
Evidence:
1. Staff #3 (Date of hire 2/13/2023) had a sworn disclosure statement dated 3/13/2023.
Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A spray bottle of perfume mist was inside a high cabinet in the Preschool 1 classroom.
Evidence:
1. Child #1 and Child #3 had diagnosed food allergies and did not have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. They had a plan for both children, but it was not from a physician.
