Inspector Notes
An unannounced monitoring inspection was initiated on August 11, 2025 and completed on August 11, 2025 in response to a self-report received by the licensing office on August 8, 2025
relating to Supervision. The inspector reviewed 1 child?s record and 3 staff records on-site on August 11, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

cited on the violation notice. Specify how the violation will be or has been corrected. Submit your
POC within five (5) business days from today, which will be the close of business on August 26, 2025. A POC submitted after this date will not appear on the public website.
Violations
3On Friday, August 8, 2025, Child A was left in a classroom, without supervision, from 9:50am to 9:54am, following a transition from the classroom to a play area off campus.
On August 8, 2025, Staff # 1, 2 and 3 were unaware that they left Child A (2 years old) alone in the classroom. There were no staff available to offer immediate assistance to the child from 9:50am to 9:54am.
On August 8, 2025, Staff # 1, 2 and 3 failed to use the center's procedure of name to face to ensure the proper counting of children in care when there is a transition to and from the classroom.
