An unannounced monitoring inspection was initiated on December 1, 2025 and completed on December 1, 2025 in response to a self-report received by the licensing office on November 25, 2025 relating to 8VAC20-780-410. The inspector reviewed 1 child?s record and 1 staff record on-site on December 1, 2025. This inspection included document review, tour of the facility, interviews, and observations.
