Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person tour of the program. A monitoring inspection was initiated on July 21, 2022 and concluded on August 3, 2022. There were 67 children present, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records and were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
8Evidence: The center director confirmed they do not have a carbon monoxide detector.
Evidence: Staff # 5?s date of hire May 12, 2022 does not have documentation of a CPS check conducted. Staff #5 was observed working on the day of the day of the inspection.
Evidence: Staff # 5?s date of hire May 12, 2022 does not have documentation of CPR / first aid training/orientation within the timeframe.
Evidence: On July 21, 2022, a center staff, (Staff 2) was observed by licensing inspector when they grabbed the upper arm of child #5 to redirect their behavior.
1. Between 11:43 am and 11:48 am Staff #2 was putting cots on the floor to prepare for afternoon rest period.
2. Child #5 was observed was running and jumping on cots.
3. Two times during this timeframe Staff #2 was observed picking up Child #5 by their left upper arm, between their elbow and shoulder to lift the child off of the cot.
4. Staff #2 told Child #5 both times ?it?s not done, you have to sit down?.
5. Staff #2 told licensing inspector (Child #5) ?he is mine? and ?he is crying because he is tired?.
Evidence: In the two?s room on July 21, 2022, center staff (Staff #2 )roughly handled Child #5 by grabbing the child by their upper arm (between their should ern ad elbow) and lifting them off of a cot.
1.Two times during 11:43 am and 11:48 am staff #2 was observed lifting up child #5 by their upper arm to remove them from a cot. The child?s feet were not on the ground when he was lifted off of the cot.
2.Child #2 screamed and cried both times they were lifted up by their arm.
Evidence: In the following classrooms there was not a staff member who obtained CPR certification;
1. In the 2?s room, staff #2 was the only staff observed with 8 children.
2. In the 18 months to 2 year old, staff #4 and staff #5 were they only staff observed with 13 children.
Evidence: In the following classrooms there was not a staff member who obtained First Aid certification;
1. In the 2?s room, staff #2 was the only staff observed with 8 children.
2. In the 18 months to 2 year old, staff #4 and staff #5 were they only staff observed with 13 children.
Evidence: The record for child # 4 has a diagnosed food allergy (milk) and the record did not include a written care plan, to include instructions from a physician.
