Inspector Notes

An unannounced complaint inspection was initiated by inspector on April 24, 2024 and completed on May 20, 2024 regarding concerns of Programs, Physical Plant, Special Service and Special Care Provision /Emergencies. During the inspection the following were completed: interviews with staff, a review of centers policies/ procedures, centers documentation, physical plant and observation of the of staff to child interactions.

The preponderance of the evidence gathered during the investigation does support the allegation related to Special Services and therefore, the complaint is determined to be valid.

Areas of noncompliance, not complaint related were cited in the area of Administration. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and staff interviews, the licensee did not ensure that when centers choose to provide meals or snacks, there shall be a menu listing foods served, to include food substitutions.

Evidence: The menu posted for April 24, 2024 listed black beans and baked beans were served instead and grapes were served and they were not listed on the menu.

Based on center documentation review and staff interviews, the licensee did not ensure to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: Child #1 was injured at the center in November 7, 2023 and the injury required outside medical treatment and the department was not notified of the injury.
