Inspector Notes
An unannounced monitoring inspection was completed today. A total of 51 children with 24 staff were onsite. Observation of the program space, classrooms, outdoor playgrounds and outside play areas was made. Staff were observed strategically positioned throughout all playgrounds to ensure adequate supervision. Children were observed outside playing, actively engaged in hide and go seek with staff and completing painting projects. A few children were planting vegetable plants next to already planted herbs. A garden path was being created using stone by staff and the children. The infant class was preparing for lunch. Staff were observed sitting on the floor next to the infants supervising while they ate. Lunch today was chicken nuggets, peas and carrots, sliced banana and bread. Both hand washing and diapering procedures were observed. A total of 3 new staff hired, all 3 records were reviewed. 3 children's records were reviewed. Medication and topical ointments currently onsite and were also reviewed. This inspection occurred approximately between 10am and 1:30pm. Thank you for your cooperation today. Staff #1 was hired on 09/27/2021. An out of state Central Registry search result has not been obtained.
Evidence:
Staff #2 was hired on 04/11/2022. Documentation of the TB screening for this staff was dated 04/15/2022.
Evidence:
1. The megaphone attached to the playground equipment with the red slides has peeling paint exposing a rusted area.
2. The surface on the platform area on the playground used for the older children is peeling posing a tripping hazard.
3. Landscape block material is exposed in some areas of the playground used for the older children creating a tripping hazard.
1. The resilient surface located under and the surrounding area of the preschool swings did not have a minimum of 6 inches of resilient surface.
2. Two wooden tree stumps were located within the fall zone trajectory of the infant/toddler swings.
3. A play house was located within the fall zone of the trajectory of the preschool swings.
