During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/24/2025 , as a part of the licensure period. The on-site inspection began at 12pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were 78 children present and staff. The inspector reviewed 7 children?s records and 5 staff records on-site. Background clearances for board officers were reviewed on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Exception: Twelve inches of space are not required where cots, beds, or rest mats are located adjacent to a wall or a divider as long as one side is open at all times to allow for passage.

Cots in one classroom were located adjacent to a wall/divider and did not allow one side to remain open at all times.
Plan of Correction: Staff will be reminded to position cots where at least one side is open
Standard 8VAC20-780-510-G
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Medication found in one classroom was not properly labeled with a child's name.
Plan of Correction: This medication will be labeled and returned to the box that it came in.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the addresses for two emergency contacts.

Child #3's record did not contain an address for one emergency contact. Child #4's record did not contain addresses for both emergency contacts.
Plan of Correction: These addresses will be obtained from the parent at pick up today.
