Inspector Notes

An unannounced Complaint Inspection was conducted today in response to a complaint that was received by the Virginia Department of Education, Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of Child to Staff Ratios, Age Appropriate Activities. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of Child to Staff Ratios, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Based on observation, the facility did not ensure that the areas inside and outside of the facility were maintained.
Evidence: Observed in the Younger Twos classroom, peeling paint and a hole in the wall large enough for a child's finger to get through.
Plan of Correction: Facilities is currently completing repair of paint and holes.
Standard 8VAC20-780-350-B
Based on observation, the facility did not ensure that staff to child ratios were maintained.
Evidence: Observed that the 4 year old classroom had a 1:11 ratio. Repeat violation: ratios were previously cited on 2/17/21 and 3/25/21.
Plan of Correction: Ensure teacher notify management if over and maintain ratios at all times.
