Inspector Notes
An unannounced Renewal Inspection was conducted on 2/14/2022 from 11:45am to 1pm. There were 145 (infants to school-age) directly supervised by 22 staff. The physical plant, 3 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, cleaning up lunch, preparing for nap, napping, participating in group and individual activities, and listening to stories. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Evidence: Reviewed 5 children's records and found that Child 3 and 4 had been in care for over 30 days and did not have documentation of a physical examination.
Evidence:
1) Observed in the Pre-K multipurpose/nap room, peeling paint under the window by the rear door.
2) Observed in the 3s classroom, chipping drywall and peeling paint.
Evidence: Reviewed 5 children's records and found that Child 1 did not have documentation of 2 emergency contact addresses or parent work information (name, address and phone).
