Inspector Notes

An unannounced Renewal Inspection was conducted on 2/14/2022 from 11:45am to 1pm. There were 145 (infants to school-age) directly supervised by 22 staff. The physical plant, 3 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, cleaning up lunch, preparing for nap, napping, participating in group and individual activities, and listening to stories. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that they obtained documentation of children's immunization records before the child attends the center.
Evidence: Reviewed 5 children's records and found that Child 3 did not have documentation of an immunization record.

Based on review, the facility did not ensure that each child had documentation of a physical examination within 30 days of the first day of care.
