Inspector Notes
An unannounced renewal inspection was conducted on 01/10/2024 from 10:55am to 2:50pm. There were 163 children ages 2 months to age 5 years old supervised by 27 staff. The physical plant, programming, attendance, menus, 5 staff records, 5 children?s records, 12 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in lunch, building, dramatic play, reading and nap. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

15Evidence: The most recent licensing inspection of the facility from the inspection on 12/21/2023, was not posted.
Evidence: The record for Staff #5 did not contain documentation that a criminal history background check report had been completed every five years from the date of the last check. The last criminal history check was completed 7/3/18.
Evidence: The record for Staff #4 (date of hire 2/27/23) did not contain documentation that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment.
Evidence: The cabinet doors in the Preschool 3B classroom (ages 3-4 years old) were broken away from the hinge and the doors were falling forward causing them to stick out in such a way as to be a risk to snagging a child?s skin from the sharp corners and a risk that the doors may fall on a child and injury them.
Evidence:
1)During inspection in the Preschool 2A classroom (ages 2-3 years old), a container of hand sanitizer was not locked up and sitting on a windowsill.
2)During inspection in the Preschool 3A classroom (ages 3-4 years old), there was an unlocked cabinet that contained 2 air fresheners, a container of sanitizer, and antibacterial cleaner.
Evidence: During inspection in the Preschool 2A (ages 2-3 years old) and Preschool 3A (ages 3-4 years old) classrooms, there were cleaning and sanitizing materials stored above paper plates and eating utensils. There were cleaning and sanitizing materials stored together with food.
Evidence:
During the inspection, the center did not maintain the required supervision ratio of 1 staff for every 4 children in the Toddler room where a child under the age of 16 months was present.
Evidence: There were records posted at the front entrance of the center dated 2/14/22, 8/22/22 and 1/5/23 that contained confidential information for staff and children.
Evidence:
In the Preschool 2A classroom (ages 2-3 years old), children did not wash their hands with soap and running water or disposable wipes before eating as required.
Evidence:
1)In the Preschool 2A classroom (ages 2-3 years old) on the diapering surface was stored children?s artwork and art supplies.
2)In the Toddler classroom, a staff member changed a child's diaper then sprayed a cleaner on the diapering surface and wiped it clean. No sanitizer was used as required. To properly sanitize the diapering surface, the sanitizer should be sprayed on the surface and the solution allowed to dry for at least 2 minutes.
Evidence:
For medications for Child #6, Child #7, and Child #8, the duration of the parent?s authorization was not renewed after 10 workdays nor was written medication authorization given from the child?s physician and parent for a long term medication.
Evidence:
The medication for Child #6 was not maintained in the original, labeled container from a pharmacy.
Evidence: In the Preschool 2B classroom (ages 2-3 years old), there were 2 diaper creams stored on the diaper station where children have access.
Evidence: There was no documentation of an evacuation drill for December 2023.
Evidence: During inspection in the Preschool 3A classroom (ages 3-4 years old), there were 16 children present, but the written record reflected there were 17 children present.
